Choose the form for the service you booked. Each one lets us evaluate your area and build your plan before you arrive at the studio.

For nano hair strokes, hybrid brows, and ombre brows. Have three clear photos of your brows in natural sunlight ready to upload.

For brows with previous microblading or PMU work that needs correcting. Uses the same brow evaluation form.

For full color lip, lip blush, ombre lips, lip liner, and dark lip neutralization.

For eyelash enhancement and wedge eyeliner. Covers contacts, lash extensions, and eye health questions.

For permanent hair removal on any area. Covers your hair-growth history and any previous laser or electrolysis.
